Video Idea, Need Help

I want to order a custom fetish video and currently I am having a hard time developing a cohesive concept with the elements I want to occur in the scene. It can be described as a mash-up of horror, wam, and DID. It also has BBW and humiliation elements.The scene involves a thin young woman entering a dining room and approaching a table. In the center of the table is a platter with a dome lid. The young woman removes the lid and sitting on the platter is the head of a beautiful female plus-size model. Her head rests on a bed of lettuce and she has an apple stuffed in her mouth, but she is alive and fully aware.

I want the young woman to do something to further humiliate the BBW, I just don't know what. I'm thinking maybe gunge, force feeding with verbal humiliation, face slapping, fat shaming? In the world of the scene this kind of thing would be normal. The young woman is not phased by the sight of the BBW's living head on a platter.
